Independent Travel Form

Please fill out this form before each weekend and every time you plan an overnight travel outside of Perugia. This will allow us to counsel you on any travel warnings and help us locate and assist you in the case of an emergency or other needs. You can also complete this form on your mobile phone via the Umbra App.

  • If you're traveling in a group, let us know. Then, only one of you will need to submit an Independent Travel Form for the whole group. Of course, if anyone plans to break off for the group, we ask that you please let us know those details as well.
  • Are you flying? Taking a train? Let us know how you plan to get around and use as many details as possible (ex. include flight number, connecting airports, etc.).
  • Name, address, city, dates of stay, phone numbers. If staying with a friend, please list a name and phone number where we can reach you in the event of an emergency.
